Target Audience
The educational design of this activity addresses the needs of community pulmonologists, allergists, emergency medicine physicians and allied health care professionals (nurse practitioners, physician assistants), nurses, community health workers and social workers.
Program Overview
In the first module, “Proactive Management of Moderate and Severe Asthma to Reduce the Impact of Exacerbations,” pulmonology and allergy experts will review the negative impact of asthma exacerbations on lung function, and the benefits and risks associated with corticosteroid and biologic treatments.
Then, pulmonologists and community health workers will discuss steps that can be taken for a patient with asthma in the emergency room to ensure appropriate referral and follow up with an asthma care provider. During “Best Practices for Care Transition to Ensure Patient Retention in Care,” experts will review the key roles of nursing staff and social workers, post-discharge processes and follow-ups, resources to encourage patient follow-up with or location of care, and strategies to retain patients in the care system.
Educational Objectives
After completing this activity, the participant should be better able to:
- Describe the negative impact of inadequately controlled asthma and repeated exacerbations on lung function, and burden of disease
- Summarize the evidence regarding the negative impacts of systemic corticosteroid exposure to control exacerbations
- Summarize the data for the efficacy, safety, and impact on exacerbations of biologics to optimize management of moderate and severe asthma
- Recognize strategies to ensure continuity of care from emergency and inpatient care settings to community HCPs for patients who presented for uncontrolled moderate or severe asthma
- Review strategies to ensure patient engagement in long-term care
