Course Description and Overall Objective
The overall objective of this program is to provide nephrologists, nephrology nurses, and other interested healthcare professionals with the most current clinical guidance on peritoneal dialysis (PD) adequacy and two of the challenging issues surrounding the delivery of PD – reduction in mechanical catheter dysfunction and the optimization of ultrafiltration management. Session 1:
Catheter Placement: Reducing Mechanical Dysfunction
PD catheter insertion can be done by open dissection via a small laparotomy incision, by percutaneous puncture guided by laparoscopy or fluoroscopy, or it can be performed blindly. This session will describe current PD access-related procedures; it will focus on advanced laparoscopic techniques, and present data on the reduction in mechanical complications associated with this approach. The session will also include a discussion of options to be used in special circumstances that might preclude initiation of PD with conventional catheter insertion techniques.

Peritoneal Dialysis Adequacy: Review of the New NKF-K/DOQI Clinical Practice Guidelines
The 2001 K/DOQI guidelines for peritoneal dialysis adequacy were developed with emphasis on urea kinetics and observational studies. Since then, randomized controlled trials have indicated the need for revision of the absolute minimal solute clearance target and weekly creatinine clearance. This
session will provide a brief history of the evolution of the NKF-K/DOQI guidelines, culminating in a presentation of the new 2006 Clinical Practice Guidelines for Peritoneal Dialysis Adequacy.

Session 3:
Optimizing Ultrafiltration Management in Peritoneal Dialysis
Optimization of fluid and ultrafiltration management remains a constant challenge in PD. This session will provide an overview of the physiologic basis for ultrafiltation in PD. It will also highlight the differences between crystalloid and colloid osmosis and the role of non-dextrose based solutions. Finally, it will review the need to identify patients at greater risk for inadequate ultrafiltration. 7:45 - 8:00 PM

- Cite the latest advances in peritoneal dialysis catheter insertion and exit-site care practices
- Discuss the merits of advanced laparoscopic techniques for the insertion of PD catheters
- Review the evolution of NKF-K/DOQI guidelines for peritoneal dialysis adequacy
- Discuss the landmark trials (CANUSA and ADEMEX) that have influenced the development of the new guidelines
- Outline the new adequacy guidelines and their implications for clinical practice
- Describe the physiology of peritoneal ultrafiltration
- Identify patients at risk for inadequate ultrafiltration
- Design a strategy for optimal ultrafiltration based on physiologic principles
